Pro-EFW Final Certification Review
You may send your complete certification package in an electronic file by email to info@equinefacilitatedwellness.org. Alternatively, you may send it by mail to the address provided on page 1. For the video submissions, you may provide You Tube links, send them via email, or mail us a USB stick.

To submit you need to be 21 years of age or older and submit the following: (all this information below is included in the downloadable checklist) 1) Details of credentials and experience in provided word document
2) Proof of 60 hours of cross training in provided word document 3) Proof of attendance at EFW Personal Development Workshop (30 hrs) in provided word document
4) A completed EFW Skills Checklist (formerly called “Horsemanship and Skills Checklist”) in provided word document including:
5) Completed and signed Mentorship Hours Form (provided pdf) 6) Completed and signed Practice Hours Form (provided pdf) 7) A current Scope of Practice statement signed by the candidate and mentor 8) A current Levels of Expertise table signed by the candidate and mentor 9) An agreement to abide by Pro-EFW's Code of Ethics signed by the candidate 10) Aditional requirement documents
11) A completed final certification form

After you have completed the Final Certification Review Form you will need to send it to your mentor for signature before submitting it with your final review documents. Do not forget to pay your final certification fees.
A CHECKLIST FOR EACH STREAM IS AVAILABLE BY CLICKING ON YOUR CERTIFICATION STREAM BUTTON FROM THE HOME PAGE
Please carefully review the Certification Requirements table for your specific area(s) to ensure you have enclosed all of the necessary documentation. You will then submit your complete certification package to the Pro-EFW Certification Review Committee.
Please note that the complete certification package must be in a file that can be emailed to info@equinefacilitatedwellness.org. (We recommend saving them in Dropbox). If you wish to send a hard copy, two complete certification packages must be mailed to the head office. The video clips are to be saved on two different memory sticks and included in the mail in package.

HOW TO SAVE YOUR DOCUMENTS
Please follow these guidelines for file and document submission:
1- Gather together all of your documents and forms.
2- Save them into one main electronic file named ‘Pro EFW Certification Application [YOUR NAME]’.
3- As you save and gather your documents, place them into the above file in the order they are requested in the Pro EFW Certification Outline. Make certain each file is well identified with the name of the document and your name.
4- Scan any certificates or documents, name them (include your name) and save them as an image (JPEG). This will make inserting them in the word documents possible. Please review the inserted scanned images to make sure that the entire document is readable. Refer to back of the page for directions on how to insert and edit images in Word.
5- Upload the video submissions that you need from the Skills Checklist as an Equine Professional or the Level 3 in the Level of Expertise to YouTube and provide the YouTube link in your documents. As soon as the video submissions are viewed by the Review Committee, the links can be removed from YouTube. If you are mailing in your submission, you can save your videos to a memory stick and send it in.
6- Once you have completed, copied/scanned and saved all of the required forms and documents, and have saved them to your main file in the order suggested, then email them as an attachment to the Pro EFW Grandparenting/Certification Committee at info@equinefacilitatedwellness.org.
7- In the subject line of your email, please write Pro EFW Certification Application [YOUR NAME]
8- You may find it easier to save and upload your files to Drop Box and send them by email in this format
Note: Please do NOT submit your application in pieces over time. Wait until you have all of your documents saved into your main file, and submit everything together at one time, in one email attachment, preferably in one pdf file or Word file.
*If you prefer to submit your application by mail, there must be two complete packages as your application is reviewed by two different people. Your video submission must be on a memory stick. The video is only if you are applying for the Equine Professional/ Equine Coach Certification or the Level 3 of the Levels of Expertise in equine experience.

How to edit the size of your image in Word:
- Place your cursor where you wish to insert an image.
- Select the “Insert” tab in the tool bar.
- Go to the “Illustrations” section and select “Pictures.”
- A window will open, and you will need to find and select the picture you wish to add.
How to edit the size of your image in Word:
- Highlight the picture by clicking on it
- Select the “Picture Format” tab in the tool bar.
- Go to “Size” section generally located to the right end on tool bar and decrease or increase the number in one box. Generally, the other box will adjust to maintain the proper proportions.
- The size of your image must not exceed the size of your page.
